Case study
One participant came into Routines for Wellbeing with upside-down sleep, isolation, and low mood. As the main carer for her son, she was often awake through the night and sleeping in the day.
Over the 6 weeks, she began to build kinder self-talk, push beyond her comfort zone, and plan small achievable actions into her day, including having a shower, getting dressed, and going out.
She described feeling better in herself, with stronger self-esteem and self-belief, and said the group helped her feel heard, more able to talk and interact with others, and more confident taking small steps forward. She also valued the peer connection, saying the group made her feel “fabulous” and that she was “winning.”

What this is
Routines for Wellbeing is a licensed, evidence-led 6-week group prevention programme designed to help adults who fall through the gaps rebuild routine, confidence, and self-management. It is built for NHS and primary care teams that want a structured response to avoidable demand, repeated short clinical episodes.
This is not signposting. It is a structured, quality-assured pathway that helps people rebuild routine and self-management with local oversight.
Most importantly, the programme aims to empower and educate without heavy clinical weighting, and with an all-important focus on the human needs of the participants in the group. Progress is individualised, but with a safety-first scripted core framework to ensure everyone feels supported and encouraged. Although there is a focus on people living with long term health conditions, the referral process is not diagnosis-dependent.
Why it matters
The bigger problem is not a lack of goodwill, it is that too many people are stuck between services with no consistent pathway to stay well in the community. RFW gives primary care and community teams a structured, repeatable way to respond to avoidable demand.

NHS England’s supported self-management evidence states that people with the knowledge, skills and confidence had 19% fewer GP appointments and 38% fewer A&E attendances than those with the lowest activation levels.

Routines for Wellbeing is designed to be delivered consistently, safely, and to a clear standard. That matters for participants, facilitators, and NHS teams alike.
This is a preventative programme delivered with fidelity, not a generic wellbeing session.

Fit check
Best for teams that want a clear, replicable model with fidelity, QA, and support. Not for teams looking for a generic wellness add-on or something they can deliver without training.
Gatekeepers: commissioners, PCN and practice leaders, and primary care teams, because they control budgets, referral flow, and pathway decisions.
Practical delivery: OTs, AHPs, social prescribers, or GPs working inside their own governance and insurance

How fidelity is maintained
