Service Description
RFW is an evidence based, group delivered preventative intervention for people with long term conditions and frequent attendance patterns who fall through the gap between short clinical episodes and real day to day recovery. In plain terms, brief appointments and medication alone do not resolve isolation or rebuild functioning. RFW provides structured routine, peer support, and behaviour change scaffolding that primary care does not have capacity to deliver, without adding work to GPs. We have seen meaningful, commissioner relevant impact grounded in validated measures and consistent participant feedback. Participant feedback is consistently strong (for example: “Can’t thank you enough, it’s been a lifeline”). We see signals of sustainability, including peer led WhatsApp support that continues beyond the end of facilitated sessions, suggesting the intervention can outlast the programme window. This programme is designed to be delivered by existing workforce (OTs, AHPs, social prescribers and GPs).
